Job summary

Heritage Health Network is looking for a Lead Care Manager in Palm Desert, CA, to support our Enhanced Care Management (ECM) program. This hybrid role allows for 2 days of remote work each week, combining flexibility with strong community engagement.

As the Lead Care Manager, you'll be the primary contact for members, develop personalized care plans, and collaborate with healthcare teams. We're seeking candidates with experience in care management and strong communication skills.

Qualifications

• Experience in care or case management is preferred.
• Knowledge of Medi-Cal and ECM is strongly preferred.
• Ability to work collaboratively in a team setting.
