Job summary

Vynca Inc. is seeking a Behavioral Health Case Manager to join our Enhanced Care Management team.

This hybrid role requires travel within Humboldt County and within 20 miles of the territory, serving as the primary contact coordinating care across providers to promote wellness and independence. Under the ECM Clinical Manager, you will develop care plans, connect clients to social services, advocate with health professionals, and document progress in EMR and CRM systems.

Qualifications

• Active, unrestricted LCSW, LMFT, or LPCC license in California required.
• 1+ year of experience as a care manager, care navigator, or community health worker; 2+ years preferred.
• Able to work Monday-Friday 8:30am-5:00pm, in the field and remotely, with flexibility for evenings/weekends.
• Knowledge of government and community resources related to social determinants of health.
• Excellent oral and written communication; positive interpersonal skills; clean driving record; valid driver's license; reliable transportation.
• General computer skills; working knowledge of Google Workspace, MS Office, and the internet.
• Bilingual English/Spanish highly preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, housing, and social supports.
• Oversees development of client care plans and goal settings.
• Offers services where the member resides, seeks care, or is most accessible (office, telehealth, field).
• Connect clients to social services and supports as needed.
• Advocate on behalf of the client with health care professionals.
• Utilize Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.
• Conduct outreach and engagement to facilitate linkage to the ECM program and log activity in CRM.
• Evaluate progress and update SMART goals; provide mental health promotion.
• Arrange transportation (e.g., ACCESS).
• Complete documentation and outcome measures within care plan timeframes.
• Maintain up-to-date EMR and other records; complete monthly reporting.
• Attend training as assigned.
