Job summary

Saban Community Clinic in Hollywood, CA is seeking a Social Services Coordinator to coordinate with members and providers, oversee ECM services, and connect members to transportation and housing under MCP guidelines. The role requires strong advocacy and trauma-informed care approaches.

The position requires a bachelor’s or master’s degree in social work or related field and at least 3 years of case management experience, plus a CA driver’s license.

Qualifications

• 3 years prior case management experience.
• Valid California driver’s license and insurance.
• Must have own transportation.
• Fluency in another language such as Spanish, Russian or Korean preferred.

Responsibilities

• Coordinate with individuals and entities to ensure a seamless experience for members.
• Oversee ECM services and Care Plan implementation.
• Connect members to social services, transportation, and housing.
• Advocate for members with health care professionals.
• Use motivational interviewing, trauma-informed care, and harm-reduction approaches.
• Coordinate with hospital staff on discharge plans.
• Accompany member to office visits as needed.
• Monitor treatment adherence (including medication).
• Provide health promotion and self-management training.
• Inform patients about other services offered by Saban Community Clinic that may be appropriate for the patient’s family members.

Salary Range: $29.61 - $33.00 Hourly

This position will work at 6043 Hollywood Blvd. Schedule is Mon-Fri. 3 days in office, 2 days remote.

Duties and Responsibilities:

• Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services
• Oversee provision of ECM services and implementation of the Care Plan
• Offer services where the member lives, seeks care, or finds most easily accessible and withinManaged care plan (MCP) guidelines
• Connect member to other social services and support needs, such as transportation and housing.
• Advocate on behalf of members with health care professionals
• Use motivational interviewing, trauma- informed care, and harm-reduction approaches
• Coordinate with hospital staff on discharge plan
• Accompany member to office visits, as needed and according to MCP guidelines
• Monitor treatment adherence (including medication)
• Provide health promotion and self-management training
• Inform patients about other services offered by Saban Community Clinic that may be appropriate for the patient’s family members.
• Performs other duties as assigned by Supervisor.

• Related work experience in public health, health outreach and education
• Fluency in another language such as Spanish, Russian or Korean preferred.
