Lead Care Manager, Bilingual (Enhanced Care Management)

This is a full-time, hybrid position that requires a mixture of field work, office work, and remote work. The primary location will be Inglewood and the surrounding area.

You may opt for a 9/80 flex schedule after six months.

The pay range is $21.38 - $24.60 per hour, with a $2,000 sign-on bonus for external candidates.

Summary

The Lead Care Manager handles care management and coordination within the ECM Program, managing cases, coordinating benefits, providing education, and facilitating access to care. The role involves collaboration with members, families, providers, and the ECM team to promote wellness and empowerment, acting as a clinical advocate and liaison.

Primary Duties
  1. Serve as the main contact for members, families, and the care team.
  2. Assess needs across health, mental health, substance use, social supports, and housing.
  3. Develop comprehensive Care Management Plans with input from the team and members.
  4. Implement care plans and health action plans.
  5. Provide services in accessible locations.
  6. Connect members to social supports and advocate on their behalf.
  7. Utilize motivational interviewing, trauma-informed care, and harm reduction.
  8. Coordinate discharge plans with hospital staff.
  9. Conduct outreach to eligible members.
  10. Attend to treatment adherence and health coaching.
  11. Arrange transportation and perform other duties as assigned.
Position Requirements
  1. Fluency in Spanish is required.
  2. Bachelor’s degree preferred or 2 years of case management experience.
  3. Ability to work with diverse populations and engage members effectively.
  4. Willingness to work in various environments including homes, clinics, and shelters.
  5. Strong communication and critical thinking skills.
  6. Proficiency with computers and documentation.
  7. Knowledge of clinical standards, community resources, trauma-informed care, and harm reduction.
