Position Summary & Responsibilities
Position Summary:
The position (K0246745)
The Kansas Department of Health and Environment has a Lead Care Coordinator opening in Topeka. The incumbent in this position will develop and monitor action plans for clients accepted into the SHCN Program for diagnostic services, direct financial assistance, and/or care coordination. The position will also lead the care coordination team to assist families in identifying, locating, collaborating, and understanding their child's health care needs in order to assure the child maintains optimal health.
Job Responsibilities may include but are not limited to the following:
Assesses family needs, confirmation of medical diagnosis and authorization for medical services, identification of client goals and objectives, action planning and assisting clients with skill development or behavior change, and evaluation of improved health outcomes to support self-sufficiency and self-determination of individuals on the program.
Determination of the client's care coordination level. This includes assigning a regional care coordinator, initiating and supporting the transfer of care coordination responsibilities to assigned regional care coordinator, and oversight of all care coordination activities for clients on the program. The level of oversight will vary depending on the level of care coordination provided.
Provide support and assistance to other care coordination staff upon request or as needed. Periodically review Action Plans developed by other care coordinators for quality assurance.
Qualifications
- Education:
- Licensing & Certification:
- Valid Driver's License - Incumbent is required to have and maintain a valid driver's license when operating a state vehicle, a private vehicle, or a rental vehicle for the benefit of the State.
- Minimum Qualifications:
- Four years of experience interviewing, compiling information, documenting decisions, interpreting guidelines and/or providing technical assistance relevant to human services programs. A Bachelor's degree in Social Work, Nursing, Public Health, or other relevant health and human service fields may be substituted for the required experience.
- Preferred Qualifications:
- Bachelor’s degree in Social Work, Nursing, Public Health, or other relevant health and human service field.
- Experience with special health care needs.
- Experience with maternal and child health genetics.
- Four or more years of experience in navigating community resources.
- Four or more years of case management or targeted case management.
- Experience with public health or other related programs.
- Teamwork/collaboration.
- Professionalism/work ethics.
- Effective oral and written communication skills.
- Proofreading, editing, attention to detail.
- Word, Excel, PowerPoint, Access.
