Job summary

NYREACH seeks a Supervisor, Certified Application Counselors, to lead Medicaid recertification and coverage-continuity efforts across Bronx programs. You will manage a CAC team, ensure accurate enrollment, and navigate six-month renewal cycles under evolving policy requirements.

You will train staff, monitor performance, and coordinate with clinical partners to maintain seamless coverage for vulnerable New Yorkers while upholding HIPAA and regulatory standards.

Qualifications

• Bachelor's degree is preferred; an associate degree with supervisory or relevant experience will be considered.
• Current NYSOH Assistor/Navigator Certification required, or obtained within first weeks of hire.
• Certified Application Counselor (CAC) certification required; additional credentials are a plus.

Responsibilities

• Lead a team of Certified Application Counselors to ensure high-quality Medicaid recertification and enrollment services.
• Supervise staff development, onboarding, performance reviews, and coaching.
• Ensure CAC staff maintain NYSOH certification and comply with HIPAA and related standards.
• Audit applications, renewals, and documentation for accuracy and timeliness.
• Set and monitor targets for completion, documentation uploads, and renewal flow.
• Coordinate caseloads, ensure coverage, and meet six-month renewal deadlines.
• Serve as escalation point for complex cases and staff guidance.
• Deliver ongoing training on Medicaid, Essential Plan, and policy changes.
• Analyze program data and prepare performance reports for leadership.
• Maintain liaison with clinical partners, LDSS, and SCN stakeholders.
• Coordinate with Health Home programs for integrated care management.
• Provide direct CAC duties during staffing gaps.
• Oversee SHOP enrollment support for small employers when assigned.
