Jewish Renaissance Foundation's JRF Community Health Center (JRF-CHC) seeks a Licensed Clinical Social Worker (LCSW) to provide behavioral health and substance abuse interventions within primary care. The role includes in-person and telehealth assessments, crisis intervention, individual and group therapy, patient education, referrals and related services.

Qualificações

• Licensed as a Clinical Social Worker (LCSW)
• Master's degree in social work required
• Two years' clinical social worker experience in healthcare preferred
• Bilingual in Spanish and English required
• Experience with behavioral health and substance abuse preferred

Responsabilidades

• Assess patients and provide individual and group therapy
• Develop personalized treatment plans
• Conduct assessments and patient surveys
• Document sessions in EMR accurately and timely
• Collaborate with multidisciplinary team and attend meetings
• Develop outreach and enrollment initiatives for services
