Job summary

Providence St Vincent Medical Center in Portland, OR seeks a Licensed Clinical Social Worker (LCSW) to independently assess, diagnose and treat or intervene for patients and their support systems. The role emphasizes evidence-informed, person-centered care across the care continuum in collaboration with an Inter/Multi-Disciplinary Team.

Requirements include an MSW (CSWE-accredited or candidacy), Oregon LCSW (or Washington LICSW) licensure, and at least 2 years of relevant social work experience.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited school or candidacy status.
• Oregon Clinical Social Worker License at hire or Washington Clinical Independent Social Worker License, with licensure in states where care is delivered.
• 2 years of relevant social work experience.

Responsibilities

• Provide biopsychosocial/spiritual assessment, diagnosis and treatment/interventions/advocacy.
• Collaborate with patient, support system and IDT to develop person-centered care plans.
• Deliver care across inpatient and outpatient populations in alignment with NASW ethics and SOPs.
