Job summary

Providence is seeking a Licensed Clinical Social Worker (LCSW) to independently provide biopsychosocial/spiritual assessments, diagnoses, and interventions for patients and their support networks. You will collaborate with the IDT to create person-centered care plans across the care continuum in both inpatient and outpatient settings within Providence PACE Oregon.

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).
• Upon hire: Oregon Clinical Social Worker License or
• Upon hire: Washington Clinical Independent Social Worker License.
• License in states wherever care is delivered.
• 2 years relevant Social Work experience. May include CSWA experience.

Responsibilities

• Provide biopsychosocial/spiritual assessment, diagnosis and treatment/interventions/advocacy.
• Collaborate with patient and support system to develop evidence-informed person-centered care plans with IDT across the care continuum.
• Work with inpatient and outpatient populations and various departments to reduce health disparities and adhere to NASW Ethics and SOP.

Education

Master's Degree in Social Work (CSWE Accredited)

Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License
