Job summary

Providence St Vincent Medical Center in Portland, OR is seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive biopsychosocial assessments and interventions across inpatient and outpatient settings. You will collaborate with an IDT to develop evidence-informed, person-centered care plans and advocate for vulnerable patients while upholding NASW ethics.

• Master's Degree in Social Work from a CSWE Accredited School, OR from a school in candidacy status by CSWE.
• Oregon Clinical Social Worker License or license in states where care is delivered.
• 2 years relevant Social Work experience (may include CSWA).

Responsibilities

• Independently perform biopsychosocial/spiritual assessments.
• Collaborate with the IDT to develop person-centered care plans across the care continuum.
• Provide diagnosis, treatment/interventions, and advocacy.
• Adhere to NASW Code of Ethics and mission while reducing health disparities.
