Job summary

Houston Methodist Willowbrook Hospital is seeking a Social Worker II (LCSW) to provide comprehensive psychosocial assessments, diagnosis and treatment planning, and complex discharge planning for patients and families on a designated unit.

This role collaborates with physicians and the interprofessional team to address the social and emotional aspects of illness and to facilitate effective transitions of care.

Qualifications

• Master's degree in social work from accredited university.
• Three years social services experience in a healthcare setting.
• LCSW license (Texas) required.

Responsibilities

• Lead psychosocial assessments, diagnosis, and treatment planning for patients and families.
• Manage complex discharge planning and care coordination across the continuum of care.
• Collaborate with physicians and interprofessional team to address social, financial, and psychosocial needs.
• Provide staff education and mentorship on psychosocial care practices.
• Document thoroughly to support safe and effective transitions of care.
