Job summary

Ambercare Hospice in Espanola, NM is seeking a compassionate PRN LCSW/LMSW to join a refreshed, collaborative team. The role focuses on psychosocial assessments, counseling, and coordinating resources to support patients and families at life's end.

Qualified candidates hold a New Mexico Master’s in Social Work and current LMSW or LCSW licensure, have at least one year in health care or hospice, and a valid driver’s license. Relocation assistance is available.

Qualifications

• Social Work degree required.
• NM MSW degree required.
• Current NM social work licensure (LMSW or LCSW).
• Minimum 1 year of health care or hospice experience.
• Valid driver's license and proof of insurance.

Responsibilities

• Performs psychosocial assessment of patients and families/caregivers in the hospice program and ensures needs are addressed in the plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
• Counsels patient and family/caregivers and staff regarding stress and coping.
• Provides accurate and timely documentation of patient services reflecting the plan of care.
• Assists with applications for SSI, Hospice, Medicaid, and Medicaid Waiver; locates resources from community agencies to meet patient/family needs.
• Liaisons between patients, families/caregivers and community agencies.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Qualifications:

• Social Work Degree
  • New Mexico: Master’s Degree in Social Work required.
• Current State Licensure in Social Work
  • New Mexico: LMSW or LCSW
• Minimum of one year’s experience in health care or hospice setting
• Valid driver’s license and proof of insurance is required
