Ambercare Hospice in Las Vegas, NM seeks a PRN Social Worker to join our newly restructured team. This role focuses on psychosocial assessment, counseling, and support for patients and families within home hospice care.

You will coordinate with the care team, assist with benefit applications, and connect families to community resources. We offer relocation assistance for qualified candidates and a comprehensive benefits package. This is a PRN, on‑call style position with flexible scheduling.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Current NM licensure LMSW or LCSW.
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Performs psychosocial assessments of patients and families in the hospice program and integrates identified needs into the care plan.
• Provides compassionate counseling to patients, families, and staff during stressful times.
• Assists with applications for SSI, Hospice, Medicaid, and Medicaid Waiver; connects families with community resources.
• Maintains accurate and timely documentation of patient services in the plan of care.
• Serves as liaison between patients, families, and community agencies.
• Keeps caseloads manageable to ensure quality patient care.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Qualifications:

• Social Work Degree
  • New Mexico: Master’s Degree in Social Work required.
• Current State Licensure in Social Work
  • New Mexico: LMSW or LCSW
• Minimum of one year’s experience in health care or hospice setting
• Valid driver’s license and proof of insurance is required
