Job summary

Yale New Haven Health's Psychiatry and Behavioral Health at Greenwich Hospital is seeking a full-time clinical social worker to treat a spectrum of psychiatric conditions in an outpatient setting, collaborating with psychiatrists, nurse practitioners, psychologists and other providers.

The role emphasizes evidence-based, solution-focused approaches, biopsychosocial assessment, treatment planning, patient and family education, and advocacy to improve outcomes.

Qualifications

• MSW from an accredited school of social work.
• LCSW licensure to practice as a clinical social worker.
• Experience in treating a broad spectrum of psychiatric conditions in a multidisciplinary setting.
• Commitment to a biopsychosocial model and program development.

Responsibilities

• Complete comprehensive biopsychosocial assessments and/or psychiatric evaluations.
• Develop and maintain a treatment plan responsive to patient/family concerns and clinical impressions.
• Engage patients and families with a patient- and family-centered approach in a plan of care.
• Provide education and counseling to individuals and families.
• Utilize evidence-based psychotherapies and crisis intervention as indicated.
• Collaborate with external providers and agencies to coordinate care.
• Refer patients to community-based services as indicated.
• Provide information and support to the interdisciplinary team and hospital colleagues.
• Participate in rounds and discussions to promote integrated care and outcomes.
• Contribute to quality assurance and performance improvement initiatives.
• Document social work activities in the EMR timely and accurately.
• Mentor/supervise and participate in projects as assigned by supervisor.
• Navigate systems and advocate for patients and families as needed.
• Engage in ongoing professional development per licensure and departmental standards.
