Job summary

The position requires a Master's in Social Work, an active LCSW license, and experience in a collaborative clinical environment.

Qualifications

• Master's Degree from an accredited school of social work.
• All candidates must have a license (LCSW) to practice as a clinical social worker.
• Experience treating adolescents in a collaborative, multidisciplinary clinical setting; Biopsychosocial approach.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and/or psychiatric evaluations.
• Develop and maintain a patient-centered treatment plan responsive to patient and family needs.
• Provide individual, couples, family and/or group psychotherapy and crisis intervention as indicated.
• Collaborate with psychiatrists, nurse practitioners, psychologists and other providers.
• Participate in interdisciplinary rounds and care discussions to improve outcomes.
• Advocate for patients and families and navigate systems for support services.
• Document timely, accurate entries in the electronic medical record.
• Mentor/supervise and participate in projects as assigned.
• Refer to community-based services and supports as indicated.
• Contribute to quality assurance and performance improvement.
