Job summary

Vital HealthCare Solutions in the Northeast seeks Licensed Clinical Social Workers (LCSWs) and Licensed Professional Counselors (LPCs) for full-time roles in geriatric mental health across skilled nursing and assisted living communities. You will deliver psychotherapy, perform evaluations, and develop tailored treatment plans in collaboration with Psychology and Psychiatry teams, providing

Qualifications

Active LCSW or LPC license (required).
Experience in geriatric mental health, long-term care, or behavioral health (preferred).
Compassionate, collaborative clinicians who thrive in interdisciplinary settings.

Responsibilities

Deliver individual psychotherapy and counseling using evidence-based practices.
Conduct comprehensive psychological evaluations and diagnostic assessments.
Develop and implement tailored treatment plans based on patient needs.
Collaborate with our Psychology and Psychiatry team, as well as facility-based medical and facility staff, to provide integrated care.
Work with a diverse population presenting a wide range of emotional and behavioral health needs.
