Job summary

Access Health Louisiana in Louisiana is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to perform assessments and provide individual, group and family therapy, coordinating care with psychiatrists and primary care physicians.

The counselor will assist with documenting case records and updating information as appropriate, supporting students' personal, social, career or educational development and addressing crisis situations as needed.

Qualifications

• Assists students in addressing obstacles affecting development (personal/social, career, or education).
• Identifies students in crisis and evaluates status; modifies care plans accordingly.
• Evaluates need for continued care or referral to other providers.
• Uses referrals for clients needing more intensive counseling or therapy.
• Reports cases such as abuse or truancy to appropriate personnel and provides counseling.

Job description

We are currently seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC). The counselor completes assessments, individual, group and family therapy, coordinates care with psychiatrist, referral sources, primary care physicians, assists with completion of all necessary case records on all assigned students, updating information as appropriate.

Essential Duties/ Responsibilities

• Performs initial assessments, evaluations, diagnoses, treatment, follow up, and education relevant to the clients care (age specific).
• Obtains histories and develops treatment plans and notes, ensuring accuracy and competency
• Acts as an advocate for the safety delivery of care and well-being of the client.
• Assists individual students in addressing obstacles that interfere with personal/social, career or educational development.
• Identifies and offers assistance to students coping in crisis. Evaluates patient status and response to treatment. Modifies plan of care based on evaluation of patient outcomes.
• Evaluates continued need for care or need for referral to other providers or support.
• Uses an effective referral process to assist students who need more intensive counseling or therapy.
• Reports special cases including child abuse and neglect, truancy, etc., to principal and appropriate personnel and provides appropriate counseling.
