Job summary

LifeLong Medical Care's Over 60 Health Center in Berkeley, CA is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Clinical Counselor (LPCC) to provide psychosocial services within an inter-disciplinary team.

Under supervision, you will conduct assessments, develop treatment plans, provide short-term counseling, case management, crisis intervention, and ensure proper documentation while supporting underserved communities.

Qualifications

• Current and valid California LCSW or LPCC license.
• Experience providing mental health services to disadvantaged populations.
• Familiarity with evidence-based practices for depression and related disorders.
• Proficient in Microsoft Office and administrative duties.

Responsibilities

• Provides comprehensive psychosocial assessments, mental health diagnoses, and treatment plans.
• Delivers short-term counseling and case management services to patients.
• Performs crisis intervention and triage as needed and coordinates with crisis teams.
• Maintains clinical and administrative records per contractual and billing requirements.
• Participates in weekly clinical meetings and supports integrated primary/mental health care.
