Job summary

LifeLong Medical Care is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Clinical Counselor (LPCC) to join the Over 60 Health Center team in the East Bay. The clinician will provide psychosocial assessments, short-term counseling, and case management within an inter-disciplinary setting to support patient care.

Qualifications

• Licensed clinical social work or counseling licensure required in California.
• Experience delivering psychosocial assessments and short-term counseling.
• Ability to work within an interdisciplinary team and coordinate care.
• Crisis intervention skills and crisis triage experience.

Responsibilities

• Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and implements short-term counseling using appropriate modalities for patients.
• Provides short-term case management services for patients participating in treatment.
• Assists with crisis intervention and triage as needed.
• Works with team to provide follow-up assessment and prevention planning after crises.
• Maintains clinical and administrative records and submits encounter forms weekly.
• Participates in weekly clinical meetings and promotes integrated primary/mental health care.
• Participates in quality improvement efforts related to behavioral health.
• Follows all State of California mandated reporting requirements.
• May supervise psychology or MSW interns and provide supervision.

Overview

LifeLong Medical Care is searching for a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), to join the team at our Over 60 Health Center. Under the general supervision of the Associate Behavioral Health Director, the LCSW/LPCC will work with an inter-disciplinary team to ensure that quality psychosocial services are delivered to health center patients.

Benefits

We offer a competitive annual salary of $101,753-$122,928 for LCSW/LMFT/LPCC, depending on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 16). Acceptance rates may vary year to year based on funding levels and the number of applicants.

Responsibilities

• Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and implements short-term counseling using appropriate modalities for patients with issues such as: depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors. Works predominantly within clinic, but may include home visit services.
• Provides short-term case management services for patients participating in treatment.
• As a team member, assists with crisis intervention and triage as needed. Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
• Works with team members to provide follow-up assessment and prevention planning after any crisis, including follow-up with outside agencies.
• Maintains clinical and administrative records in accordance with contractual and billing requirements and submits encounter forms on a weekly basis.
• Participates in weekly clinical meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
• Participates in quality improvement efforts related to behavioral health and integrated models of care.
• Consults informally with providers as needed.
• Follows all State of California mandated reporting requirements.
• May serve as supervisor for psychology or MSW interns and may provide ASW or AMFT or APCC clinical supervision.
