Chapa-De is a non-profit community outpatient health center dedicated to advancing the health and well-being of American Indians and low-income individuals. As a Behavioral Health Therapist (LCSW), you will provide integrated behavioral health care alongside primary care providers in a collaborative, whole-person wellness model.

This full-time position offers a salary of $91,400 to $106,050 annually, a sign-on bonus and relocation for eligible new hires, and a strong benefits package including

Qualifications

• LICSW licensure required.
• At least 3 years of experience in a Behavioral Health Program.
• Clinical experience with adults, families, and children.
• BLS certification.

Responsabilités

• Provide initial patient evaluations, triage assessments, crisis intervention management, brief individual and group counseling, skills training, care coordination, case management and follow-up referrals.
• Participate in interdisciplinary case conferences, work with primary care providers and assist with program development.
