Qualifikationen

• Master's Degree in Social Work from a CSWE Accredited School of Social Work or candidacy status by CSWE.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License upon hire.
• Active license in states where care is provided.
• Minimum 2 years of relevant social work experience.

Aufgaben

• Provide biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
• Collaborate with patients and support systems to develop evidence-informed person-centered care plans across the care continuum with the IDT.
• Work with inpatient or outpatient populations in collaboration with various departments.
• Adhere to NASW Code of Ethics, Standards of Practice and Providence mission.
