Formación

• Familiarity with HIPAA and patient rights policies.

Responsabilidades

• Provide direct social work services and counseling to patients and families to enhance psychosocial well-being.
• Assess patient needs within 72 hours of admission to inform a written social work plan.
• Notify physicians of significant changes in patient condition and coordinate with care teams.
• Coordinate with medical staff on discharge planning, referrals, and social support services.
• Maintain accurate documentation and participate in interdisciplinary reviews and trainings.
