Parkland Community Health Plan is seeking a Behavioral Health Social Worker-LCSW for night shifts in the Main Hospital Building. You will perform psychosocial assessments, diagnose, and implement care plans for patients in a hospital setting.

The role requires an LCSW licensed in Texas, CPR within 60 days, and SAMA certification within 30 days. You will provide short-term therapy, coordinate resources, and ensure documentation complies with confidentiality and regulations.

Qualifikationen

• Master's degree in social work is required.
• Licensed as an LCSW in Texas by the Behavioral Health Executive Council (BHEC).
• Current Heartsaver CPR within 60 days of hire and/or placement. CPR certification must be from American Heart Association / American Red Cross / Military Training Network.
• Satori Alternatives for Managing Aggression (SAMA) certification, level 1-4, must be obtained within 30 days of hire or placement.

Aufgaben

• Conduct psychosocial assessments to determine diagnosis and patient needs.
• Provide short-term therapy and refer to resources for psychosocial issues (transportation, equipment needs, financial, housing, legal).
• Develop and implement a care plan aligned with patient needs, risks, and support.
• Identify and resolve crises with patients/families; promote empowerment.
• Develop and facilitate health education classes on prevention and wellness.
• Handle referrals from physicians, nurses, staff and community agencies; maintain knowledge of eligibility and intake processes.
• Participate in social work meetings and reviews; seek supervisory guidance to improve methods.
• Maintain thorough written records of counseling sessions and confidentiality compliant documentation.
• Compile accurate statistical information for reports in a timely manner.
• Stay updated on social work trends through conferences and professional involvement.
• Ensure knowledge of applicable laws and guidelines; promote adherence and internal controls.
• Identify process improvements and tailor interactions to sensory needs of all customers.
• Serve as LPHA as defined by Texas Administrative Code for Behavioral Health services.
