Job summary

Strive Health is seeking a Licensed Professional Counselor (LPC) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LPC will conduct assessments, deliver individual and group therapy, collaborate with the interdisciplinary team (IDT) and ensure regulatory compliance.

Responsibilities include developing treatment plans, delivering therapy, documenting sessions per Medicare and facility guidelines, and supporting residents and families through

Qualifications

• Master’s in counseling or a related field is required.
• Enrollment or eligibility for Medicare Part B provider is required.
• Experience with geriatric populations or long-term care is preferred.
• Strong interpersonal, documentation, and communication skills.
• Ability to work independently within a team-based environment.

Responsibilities

• Conduct initial assessments and develop treatment plans.
• Provide psychotherapy services tailored to residents.
• Deliver individual, group, and family counseling.
• Document sessions in compliance with Medicare and facility requirements.
• Participate in care plan meetings and interdisciplinary team discussions.
• Support residents and families in coping with illness and placement.
• Collaborate with Strive psychiatric nurse practitioners and facility staff.
• Educate facility staff on recognizing psychosocial needs and appropriate referrals.
