Job summary

BrightSpring Health Services in Denver is hiring Licensed Clinical Social Workers (LCSW) to support home and community neuro rehabilitation services. You’ll perform comprehensive evaluations, set measurable goals with the rehab team, and coordinate care to maximize functional recovery in the patient's natural environment.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Active LCSW licensure in the practicing state required.
• At least 1 year rehab experience with brain/spinal injuries or stroke.
• Current BLS/CPR certification required.
• Ability to lift up to 100 pounds with safety procedures.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social/emotional factors.
• Counsels patients and families on coping with illness.
• Participates in care coordination and resource identification.
• Identifies community resources to meet goals.
