Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Clinical Social Worker (LCSW) to join our integrated clinical team onsite at one or more long-term care facilities in your community.

You will provide therapy, assessments, care planning, and behavioral health support for older adults and medically complex residents, with a Monday–Friday schedule and robust administrative support.

Qualifications

• Active, unrestricted LCSW license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Comfortable navigating LTC environments.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical staff to coordinate care.
• Maintain complete and timely documentation using the GNT.
• Build strong partnerships with facility staff and leadership.
