Job summary

Transformation-House in Fridley, MN is seeking a Licensed Alcohol and Drug Counselor (LADC) or ADC-T for our men’s residential program. This temporary role offers a meaningful opportunity to transform lives through recovery work.

You will conduct substance use assessments, provide case management and individual sessions, facilitate group therapy, perform the 12 core counseling functions, and maintain precise documentation. A DHS background check and Procentive EMR experience are preferred.

Qualifications

• Current MN LADC license or ADC-T temporary permit
• Ability to pass DHS background study
• Experience with EMR (Procentive preferred)
• Basic computer skills
• Must provide proof of 12 hours of co-occurring training at hire and 8 hours annually

Responsibilities

• Conduct comprehensive substance use assessments
• Provide case management and individual sessions
• Facilitate therapeutic group sessions
• Perform the 12 core counseling functions
• Utilize the Minnesota Matrix Six Dimension model
• Maintain accurate, timely documentation
• Collaborate in weekly multidisciplinary team meetings
• Documentation, ITP, TPR
