Job summary

Kalele Care Services on Hawai'i Island seeks a compassionate Community Health Worker to support kupuna with transition of care, health education, and access to services. You will deliver in-person care in hospital, rehab, and community settings, with some telephonic support, coordinating with healthcare providers and family members.

The role emphasizes cultural sensitivity, respect, and empowering patients to manage their health and navigate resources, with opportunities for growth within Ohana

Qualifications

• High school diploma or equivalent.
• Associate or Bachelor’s degree in social work or other healthcare related field, community health worker certificate, or nurse aide training preferred.
• Experience in healthcare or home health field or work with the geriatric population, preferred.

Responsibilities

• Support services primarily in-person in hospital and short-term rehabilitation settings; in the community, in person and telephonically as needed.
• Assist with treatment goals or care plans created by APP and improve healthcare access.
• Collaborate with APP to develop patient goals related to wellness, access to care, and self-management.
• Respond to patient inquiries and assist with paperwork; facilitate remote visits via telephone or video.
• Educate patients on care processes and help reduce anxiety and fear; identify barriers to care and refer to resources.
