Job summary

Ohana Pacific Health seeks a Community Health Worker to serve on Kaua'i, with care focused on kūpuna in person at hospital and rehab sites and via telephone where possible.

You will help patients access services, support care plan goals with APPs, and assist with paperwork. This role emphasizes empathy, cultural sensitivity, and community navigation to improve health outcomes for Hawaii's Kupuna.

Qualifications

• High school diploma or equivalent.
• Associate or Bachelor’s degree in social work or other healthcare related field, CHW certificate, or nurse aide training preferred.
• Experience in healthcare or home health field or work with the geriatric population, preferred.

Responsibilities

• Assist patients in navigating and accessing health and community services.
• Support in carrying out treatment goals or care plans created by APP.
• Facilitate remote visits between patient and provider using telephone or video conferencing; assist with paperwork as needed.
• Identify patient barriers to care (financial, transportation, insurance, language) and refer to additional resources.

Education
