Job summary

US Renal Care is seeking a Kidney Care Options Educator, Care Coordinator to promote and deliver the Kidney Care Options Program across the McAllen region and surrounding areas. You will educate nephrology offices, hospital staff, and patients with CKD/ESKD, coordinating care and supporting home therapy growth.

Under RN supervision, you will track metrics, maintain documentation, and collaborate with clinicians, administrators, and community partners to drive referrals and improve outcomes.

Qualifications

• Bachelor's degree or equivalent work experience.
• Five years of experience in a healthcare organization.
• Knowledge of nephrology and ESRD/dialysis required.
• 1 year patient education and/or 1 year sales experience preferred.
• Bi-lingual preferred; able to communicate effectively.

Responsibilities

• Promote and deliver the Kidney Care Options Program to nephrology offices, hospital case managers, and USRC staff.
• Educate patients with CKD and ESKD and coordinate care for dialysis modalities.
• Track and report program metrics; maintain documentation and communication across teams.
• Build and sustain relationships with external and internal physicians/hospitals.
• Coordinate care including home therapy growth and patient transitions.
