KHC/COMMUNITY CARE PHARMACIST (Finance)
Provide patient-centered services that optimize medication therapy for improved clinical and economic outcomes and excellent patient care from a centralized contact center. Services will be provided telephonically and include, but are not limited to: Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, and other Enhanced Patient Education Services. Ensure accurate and efficient KHC service is provided to our patients, the pharmacies we service, and the providers we support. Minimum Qualifications
- Pharmacy Degree (BS-Pharmacy/PharmD) Required
- Must be licensed and remain in good standing with the State Board of Pharmacy in resident state
- Must be willing and able to obtain additional non-resident licensures as assigned
- ASHP-accredited residency or 2+ years relevant pharmacy experience
- Proven strength in written and oral communication skills to effectively relay information to patients, pharmacies, providers, and all levels of the organization
- Effectively able to prioritize and handle multiple projects and responsibilities
Desired Qualifications
- Pharmacy Board Certification (Board of Pharmacy Specialties)
- 2+ years MTM experience
- Demonstrated leadership and coaching skills
Responsibilities
- Identify, prioritize, and resolve medication-related problems, determine appropriate follow-up care, and prevent new medication therapy problems
- Help patients identify barriers to adherence and develop solutions to overcome those barriers
- Use appropriate and professional communication skills and effective education techniques to engage patients and/or caregivers in decision-making
- Formulate therapeutic goals for patients or caregivers based on medications and disease states using knowledge of primary literature and guidelines
- Make professional recommendations to other healthcare team members clearly, concisely,, and in a timely manner
- Display cultural competence, respect for diversity, and respect for patient preferences and needs when providing patient care
- Teach, mentor, and supervise pharmacy students during IPPEs and APPEs
- Precept and mentor other staff, interns, or residents
- Ensure patient information and all practice areas are handled in a compliant manner
- Meet productivity and performance expectations set forth by supervisor
- Demonstrate understanding and commitment to Kroger Health’s purpose, vision, and mission
- Apply effective workload and time management skills and document all services promptly
- Assist supervisor with other projects as needed
- Support other Kroger Health operations when required
- Work cooperatively in a high-paced and sometimes stressful environment
