Job summary

Sanctuary Hospice is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) to join our nursing team delivering exceptional end‑of‑life care in the Cincinnati community.

This full‑time position offers a Monday–Friday, 8 am–4:30 pm schedule, with a focus on compassionate, patient‑centered care and strong collaboration with the interdisciplinary team.

Qualifications

• Holds current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp is a plus.
• Proficient with computers and EMR software.
• CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise, addressing inquiries compassionately.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct RN case management to ensure comfort and quality of life.
• Deliver top‑notch care rooted in proven nursing principles.
• Educate and advocate for patients and families in creating personalized care plans.
• Share call duties as part of on‑duty coverage.

Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$80,000 - $92,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
