Job summary

CN Guidance & Counseling Services, Inc. is seeking a Care Coordinator to join an interdisciplinary team providing care coordination for severely mentally ill adults with complex medical needs.

The role involves advocacy, collaboration with community partners, and ensuring access to services to improve wellness and reduce ER visits or hospitalizations. Educational requirements include a Bachelor's or Master's degree in a related field.

Qualifications

• Bachelor's or Master's degree in a related field is preferred.
• Experience coordinating care for mentally ill or medically complex clients is preferred.
• Working knowledge of computer software and EHR systems; strong documentation skills.

Responsibilities

• Conduct initial and ongoing client assessments to identify strengths, needs, goals, and resources.
• Develop and update client-centered, integrated care plans in collaboration with the team.
• Maintain communications with clients, physicians, and care providers to ensure access to services.
