Job summary

Omnicare, supported by CVS Health, seeks a licensed pharmacist to oversee IV therapy within long-term care and home infusion settings. This role emphasizes clinical management of patients, ensuring regulatory compliance and accurate documentation.

The position requires a Pharm.D. or Pharm.D./BS with infusion experience, a current license, and strong collaboration with facilities and IV teams. Work schedule aligns with facility needs and infusion service operations.

Qualifications

• 3 years infusion therapy experience which MUST include clinical management of patients.
• Current license to practice pharmacy in good standing.
• Bachelor's Degree or Pharm.D. from an accredited School of Pharmacy.

Responsibilities

• Ensure compliance with all pharmacy laws and regulations, and sterile compounding guidelines.
• Maintain all required IV documentation and clinical monitoring.
• Assess new patients and orders for appropriateness of therapy and safety.
• Dose calculations based on medication, patient condition, and administration rates.
• Program infusion pumps and monitor infusion patients.
• Oversee training and competency for pharmacists and technicians rotating to IV.
