We are currently seeking responsible, caring, and dedicated individuals to fill ISO Support Service Worker positions within our Home and Community Services (HCS) program across Washington County.

The ISO Support Service Worker plays a vital role in fulfilling our mission by providing care and supervision, supporting activities of daily living, and engaging in social and recreational activities. Responsibilities include ensuring the safety and well-being of individuals, serving as positive role models, and transporting individuals using personal vehicles when appropriate.

Duties and Responsibilities:

1. Perform tasks outlined in the individual’s Individual Plan (IP).
2. Complete assigned tasks as requested.
3. Provide services within the authorized scope of the IP.
4. Deliver services in accordance with Passavant Memorial Homes policies and procedures.
5. Supervise and serve as a positive role model at home and in the community.
6. Complete necessary Pre-Service and Annual In-Service training.
7. Observe and report on individuals' behaviors and physical condition.

Required Skills and Knowledge:

1. Must be at least 18 years old.
2. Valid driver’s license required.
3. Current car insurance and registration required (personal vehicle use).
4. Successfully pass background check.
5. Experience working with individuals with developmental disabilities is preferred.
