Lutheran Social Services of Wisconsin & Upper Michigan (LSS) seeks an IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program. This position combines elements of case management, care coordination, social work, disability services, long-term care, and community-based support to help participants maintain independence, achieve personal goals, and direct their own services.
As an IRIS Consultant, you will build meaningful relationships with participants, coordinate services, promote self-determination, and ensure health and safety while supporting participants in living the life they choose.
