Job summary

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) seeks an IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program. The role blends case management, care coordination, social work, and long-term care in a community-based setting.

You will build relationships, coordinate services, promote self-determination, and ensure safety while helping participants live independently and access needed resources.

Qualifications

  • Bachelor-level degree in a relevant field and at least one year of supervised experience working with older adults or individuals with disabilities.
  • Alternatively, four (4) or more years of direct experience providing services to older adults or individuals with disabilities.
  • Possession or eligibility as an IRIS Consultant under Wisconsin DHS exemption is acceptable.

Responsibilities

  • Develop and maintain trusted relationships with IRIS participants.
  • Assist participants in creating and updating individualized IRIS Support and Services Plans.
  • Conduct monthly participant contacts and required quarterly in-person visits.
  • Promote participant choice, self-direction, independence, and community inclusion.
  • Monitor participant health, safety, and program outcomes.
  • Educate participants about IRIS enrollment requirements, eligibility, and available resources.
  • Collaborate with ADRCs, healthcare providers, and service providers.
  • Maintain accurate case documentation within required systems.
  • Complete documentation within compliance standards and timelines.
  • Assist with annual Long-Term Care Functional Screens when applicable.
  • Work independently while collaborating with an interdisciplinary team.

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate, organized, and self-motivated IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.

This position combines elements of case management, care coordination, social work, disability services, long-term care, and community-based support to help participants maintain independence, achieve personal goals, and direct their own services.

As an IRIS Consultant, you will build meaningful relationships with participants, coordinate services, promote self-determination, and ensure health and safety while supporting participants in living the life they choose.

What You'll Do

As an IRIS Consultant, you will:

  • Develop and maintain trusted relationships with IRIS participants

  • Work independently while collaborating with an interdisciplinary support team
Qualifications
Required

One of the following:

  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Sociology, Special Education, Nursing, Rehabilitation Services, Healthcare Administration, or a related field and at least one year of supervised experience working with older adults or individuals with disabilities

OR

  • Four (4) or more years of direct experience providing services to older adults or individuals with disabilities

OR

  • Current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption
