Job summary

Provident Behavioral Health in Wisconsin is seeking a compassionate IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program. The role emphasizes person-centered planning and collaboration with care teams.

You will conduct monthly contacts, update plans, promote independence, and ensure accurate documentation while traveling within the service area.

Qualifications

• Bachelor's degree in Social Work or related field with at least one year of experience working with individuals with disabilities or older adults, or four or more years of relevant experience, or current IRIS Consultant status.
