Job summary

MercyFirst in Brooklyn, NY is seeking a Therapeutic Case Manager for its Safe Way Forward program. The role combines home- and community-based case work, crisis intervention, safety planning and coordination of services for families affected by IPV, with a trauma-informed, family-centered approach.

The position requires an MSW or related field and experience with children and families in the child welfare system.

Qualifications

• MSW or related Human Services field is required.
• Experience with children and families known to the Child Welfare system is required.
• Experience with families affected by IPV preferred.
• Demonstrate cultural competence and commitment to anti-racism work.
• Strong engagement, organizational and communication skills.
• Understanding and commitment to the agency’s mission and Sanctuary commitments.

Responsibilities

• Community-based, home-based case work counseling, crisis intervention, safety planning, advocacy, and referrals within program standards.
• Maintain caseload of 10–11 families, rotating primary client among Survivor/child or PCH to build skills.
• Participate in weekly Clinical Team rounds to align services with family needs.
• Facilitate Family Team Conferences to build consensus on Service Plans.
• Support implementation of TF-CBT and AF-CBT models.
• Complete timely Progress Notes, Family Assessments and Service Plans.
• Maintain case documentation in systems of record and be on-call for emergencies.
• Participate in supervision, meetings and trainings to advance program development.
• Foster a respectful, inclusive workplace and collaborate with external stakeholders.
