Job Scope

Responsible for providing employment services to ACT consumers, such as job development and job coaching. The role involves 50% community scouting for employment opportunities and 50% providing individual and group counseling to individuals entering or preparing to enter the workforce. The position requires working on a multi-disciplinary team to deliver comprehensive employment services, including skills acquisition.

Essential Functions

• Provides employment services to consumers.
• Participates in assessment and treatment planning, focusing on employment goals.
• Provides counseling to reduce barriers and develop skills.
• Maintains case records, including progress notes and treatment plans.
• Inputs case-related information into relevant systems.
• Participates in team meetings to discuss consumers.
• Develops community linkages and support services.
• Builds partnerships with potential employers.
• Facilitates rapid job placement.
• Participates in training sessions and assignments with CPI.
• Enters consumer employment interest data into NYESS.
• Maintains employment records in agency systems.
• Provides ongoing support post-placement.
