CORA is in search of an Intensive Prevention Services (IPS) Case Manager to work with our clients and families receiving services.

Intensive Prevention Services (IPS) is a program rooted in strong clinically minded care and social justice. Many of our client referrals come through a police diversion program which gives youth ages 10-19 the opportunity to participate in counseling rather than be arrested for minor offenses such as theft, possession, or minor assault. Our IPS Case Manager works in collaboration with the IPS team, appropriate social service agencies, court personnel, and other significant persons in implementing service plan goals.

Starting rate: $20.75

*Rate is commensurate with credentials and experience.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Engage clients, their families, and other significant persons in a collaborative relationship that will enhance the potential for the client’s completion of service with positive outcomes.
  2. Work collaboratively with members of the IPS team to implement the service plan goals.
  3. Assist with group programming with an emphasis on resiliency skills. Assist other IPS cohort with group programming, as necessary.
  4. Maintain an ongoing awareness of availability of community resources and facilitate client/family access to appropriate resources and services necessary to support and supplement service goals.
  5. Provide case management services to clients enrolled in both group and Targeted Intervention in community settings most conducive to achieving desired goals with clients. Settings include but are not limited to clients’ homes, schools, court, recreation centers, and treatment facilities.
  6. Participate in regular team consultations with CORA and DHS/Court staff to provide service updates and collaborative planning.
  7. Participate in all necessary interdisciplinary functions and meetings.
  8. Assist with transportation of clients.
  9. Document client services in designated electronic databases.
  10. Uphold the mission, vision, and values of CORA Services, which includes valuing all individuals, supporting the individual’s potential for growth, and enhancing a programmatic service system approach.
  11. Perform other duties as assigned by IPS Leadership.

EDUCATIONAL REQUIREMENTS:

  • Bachelor’s level degree in one of the helping professions.

EXPERIENCE/KNOWLEDGE/SKILLS:

  • Documented case management experience or experience working with youth in a group counseling setting.
  • Experience providing direct service in community locations preferred.
  • Experience working with at-risk youth preferred.
  • Ability to work evening hours.
  • Valid Driver’s License.
