Job Description

The full-time Bi-lingual IPP Case Manager reports to the IPP Case Management Supervisor. This role involves working directly with participants who are court-stipulated for treatment services, under the oversight of the Adult Probation and Parole departments. The primary responsibilities include ensuring services are provided and documented according to DAP, CBH, Forensic Services, and PHMC standards. The case manager will support participants and collaborate with management within the treatment program, Forensic Services, and the recovery community.

Responsibilities

  • Establish and maintain variable service productivity targets based on program needs.
  • Provide case management to chemically dependent/co-occurring individuals, managing a caseload of about 25 participants, primarily bilingual Spanish speakers.
  • Document services within 24 hours of delivery.
  • Participate actively in team meetings, case conferences, and trainings.
  • Collaborate with court personnel, probation/parole, treatment providers, and other agencies.
  • Assist individuals in linking to recovery support services and community resources.
  • Maintain weekly work schedules with detailed activities using the program's Outlook feature.
  • Keep participant records compliant with agency standards and licensing requirements.
  • Help participants enroll in treatment, health, vocational, and educational services.
  • Coordinate with community resources to promote recovery-oriented services.
  • Communicate effectively within and outside the agency to foster teamwork.
  • Spend approximately 95% of work time in the community for appointments and meetings.
  • Flexible schedules may be available to provide services during non-traditional hours, based on needs.

Skills

  • Valid driver’s license required.
  • Excellent customer service and communication skills.
  • Ability to work in the community using public transportation.
  • Strong time management, analytical, and problem-solving skills.
  • Fluent in Spanish, both spoken and written.

Experience

  • Experience in case management with chemically dependent criminal justice populations is a plus.

Education

  • Minimum an Associate’s degree in social services, sociology, psychology, criminal justice, or social work, with 2 years of relevant experience, including at least 1 year in addiction recovery, mental health, or behavioral health settings.
  • Bachelor’s degree preferred, with relevant coursework and participant contact experience.
