Job description

JOB DESCRIPTION:

The full-time, bilingual IPP Case Manager reports to the IPP Case Management Supervisor. This role involves working directly with participants who are court-stipulated for treatment services, under the oversight of the Adult Probation and Parole departments. The primary responsibilities include ensuring services are provided and documented according to DAP, CBH, Forensic Services, and PHMC standards. The individual will support participants while collaborating with management within the treatment program, Forensic Services, and the recovery community.

Responsibilities:

  1. Establish and maintain variable service productivity targets based on program needs.
  2. Provide case management to chemically dependent/co-occurring individuals, managing a caseload of approximately 25 active participants, primarily Spanish-speaking.
  3. Document services within 24 hours of delivery.
  4. Participate actively in team meetings, case conferences, and trainings.
  5. Collaborate with court personnel, probation/parole officers, treatment providers, and other agencies.
  6. Assist participants in accessing recovery support services, including Pro-ACT, Access to Recovery, faith-based institutions, mental health services, and community supports.
  7. Update weekly work schedules with detailed activities using the program's outlook feature.
  8. Maintain participant records in compliance with agency standards, managed care, and licensing requirements.
  9. Prioritize enrollment in treatment, health services, DPA, vocational, and educational services.
  10. Identify and coordinate community resources to enhance access to recovery-oriented services.
  11. Coordinate and communicate with internal and external programs to promote teamwork.
  12. Spend approximately 95% of work time in the community for appointments and meetings.
  13. Flexible work schedules may be available to provide services during non-traditional hours, based on needs.

Experience:

  • Experience in direct case management with the chemically dependent criminal justice population is a plus.
  • Experience in fee-for-service delivery is a plus.

Education:

  • Minimum of an Associate’s degree in social services, sociology, psychology, criminal justice, or social work, with 2 years of experience in human services, including 1 year in addiction recovery, mental health, or behavioral health settings.
  • Bachelor’s degree preferred, with coursework in sociology, social work, psychology, anthropology, political science, history, criminal justice, counseling, or education, and at least 1 year of direct participant contact in relevant settings.
