Job summary

Heartland Family Service is seeking a Program Coordinator for the Iowa ACT program, based in Council Bluffs. You will oversee ACT teams, ensuring fidelity to the ACT model while coordinating with clinical staff and partners to deliver integrated, community-based care.

The role demands 3–5 years with supervisory experience, strong organizational skills, and expertise in Medicaid and behavioral health documentation. Expect a 40-hour week, with some evenings/weekends as needed.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field; Master’s degree preferred.
• Experience with ACT model principles and fidelity standards is required or preferred.
• Working knowledge of Medicaid billing, behavioral health documentation, and payer requirements.
• Excellent organizational and communication skills; ability to manage multiple priorities.

Responsibilities

• Lead day-to-day ACT program operations with fidelity to the ACT model and accreditation standards.
• Track program performance metrics and client outcomes across ACT Programs at HFS.
• Manage Council Bluffs ACT Team schedules, workflows, and on-call coverage.
• Collaborate with clinical staff on intake, transitions, and discharge planning with community partners.
• Ensure documentation and records are accurate and compliant; support billing and coding processes.
