Horizon Health is seeking an IOP Therapist for UnityPoint Health-Finley Hospital in Dubuque, IA. The role supports day-to-day clinical operations, including assessments, group and individual therapy, and treatment planning in a behavioral health program.

The position requires a Master’s degree in social work or clinical counseling and an IA license (LMSW/LCSW/LMFT/LPC) with 1–3 years of related experience. Collaboration with interdisciplinary teams and thorough documentation are essential

Qualifications

• Masters degree in social work or clinical counseling required.
• Active IA clinical license (LMSW/LCSW/LMFT/LPC) or equivalent required
• 1-3 years of related working experience

Responsibilities

• Curriculum development and implementation
• Referral management, assessments and direct client contact (group, individual and family therapy)
• Implement and manage systems to ensure effective communication with clients, families, referral sources and other relevant entities
• Treatment, discharge and safety planning
• Thorough and timely clinical documentation
• Active participation in clinical consultation, supervision and collaboration
• Other duties as assigned
