Freedom Behavioral Hospital is seeking a qualified IOP Program Therapist to join our multidisciplinary team in Cleveland, MS. This role delivers high-quality behavioral health services in an outpatient setting, including group, individual, and family therapy, while supporting patients through the continuum of care.

The Program Therapist participates in treatment planning, case management, and discharge coordination to ensure patients achieve optimal clinical outcomes and successful reintegration

Qualificações

• Master’s degree in Social Work, Counseling, or related behavioral health field.
• Mississippi licensure as LCSW, LMSW, or LPC.
• Clinical experience in behavioral health is preferred.

Responsabilidades

• Provide group, individual, and family therapy in an IOP setting.
• Develop, implement, and update individualized treatment plans.
• Complete psychosocial assessments and clinical documentation in accordance with regulatory standards.
• Coordinate discharge planning and connect patients with community resources.
• Collaborate with the multidisciplinary treatment team (physicians, nursing, case management, etc.).
• Ensure compliance with CMS, Joint Commission (JCAHO), and facility policies.
• Maintain timely, accurate, and complete documentation in the medical record.
• Participate in team meetings, case reviews, and performance improvement initiatives.
