Qualifications

Responsibilities

• Assess patient physical, psychological, social, and discharge planning needs.
• Formulate a plan of care and discharge plan in collaboration with patient/family.
• Implement the plan of care with direct patient care and coordination.
• Evaluate patient responses and update the care plan as needed.
• Document assessments, plans, and outcomes in the patient record.
• Provide care based on physician orders and nursing plan of care, ensuring policy compliance.
