Job summary

Johns Hopkins Personalized Primary Care (JHPC) in Maryland seeks a BC Internal Medicine Primary Care Physician to join a full-time panel of 550 patients, delivering deeper relationships, more one-on-one time, and highly personalized care plans tailored to each patient’s history and health goals.

You will coordinate care with Johns Hopkins specialists, provide comprehensive preventive and chronic disease management, extend access through longer visits and proactive outreach, and utilize

• Board certification in Internal Medicine.
• MD license in Maryland.
• CPR certification.

Responsibilities

• Provide comprehensive primary care to a full-time panel of 550 patients.
• Deliver individualized care plans based on medical history, prevention, chronic disease management, and patient goals.
• Offer enhanced access and continuity through longer visits, proactive outreach, and collaboration with a dedicated care team.
• Utilize telehealth and other digital tools to maintain timely communication and follow-up.
• Coordinate care with Johns Hopkins specialists and health system resources as needed.
