Job summary

HealthPartners is seeking a full-time board-certified physician to join our Advanced Care Primary Care clinic at the HealthPartners St. Paul Clinic and Park Nicollet St. Louis Park Clinic.

This outpatient role focuses on patient-centered, team-based care for rising and moderate risk patients with an emphasis on improved health outcomes and affordability. You will work within the largest multi-specialty care system in the Twin Cities, delivering resources and coordinated care in a mission-driven

Qualifications

• Must be board-certified in family medicine, internal medicine, or internal medicine/pediatrics.
• Outpatient primary care experience preferred.
• Strong communication and teamwork in a care coordination model.

Responsibilities

• Provide outpatient primary care to rising and moderate risk patients.
• Collaborate with care team to improve health outcomes and affordability.
• Participate in clinician well-being programs and CME activities.

Job description

HealthPartners is seeking a full-time board-certified Family Medicine, Internal Medicine or Internal Medicine/Pediatrics physician to join our new Advanced Care Primary Care clinic located at the HealthPartners St. Paul Clinic and Park Nicollet St. Louis Park Clinic. This new care model in primary care is designated to provide patient-centered and team-base care for rising and moderate risk patients with a focus on improved health outcomes and affordability. This is an outpatient role where you will provide patients with more resources and greater coordination of care. PAY RANGE

$237,616.00 annually for 0.8FTE to $297,020 annually for 1.0FTE

Compensation is based on the level and requirements of the role. Pay within our ranges may also be determined by education, experience, knowledge, skills, location, and abilities as well as internal equity.
