We're a Social Primary Care practice serving clinically and socially complex older adults in underserved Boston communities. Our model gives you the time, the team, and the tools to actually treat the whole person not just their chart.

Panel size is capped at 750 patients.

• Medical Technician Partners (EMTs/Paramedics) deployed into patients' homes for post-discharge follow-ups, chronic condition monitoring, and home safety assessments, reporting directly back to you


• Medical Assistants, Patient Benefits Partners, and Practice Operations Partners handling the administrative and clinical support so you can focus on care

A typical week

• Clinic and telehealth visits with your panel (no double-booking, no 10-minute slots)


• Care team huddles with Guides and Medical Technician Partners


• Remote administrative time for documentation, care coordination, and team communication


• Home-based care coordination supported by your MTP team (you direct; they deploy)
