Job summary

LaSante Health Center in Brooklyn is seeking an Internal Medicine Provider to join our multispecialty, community-based health center. The role focuses on comprehensive primary and preventive care for adults, managing chronic conditions and promoting wellness in a culturally diverse community.

Creole-speaking candidates are strongly preferred, and the successful clinician will work with a multidisciplinary team to deliver patient-centered care and maintain accurate EHR documentation.

Qualifications

• MD/DO/NP/PA from an accredited institution.
• Board certification or eligibility in Internal Medicine or Adult Primary Care.
• New York State license or eligibility.
• Experience in community health or outpatient primary care preferred.
• Excellent communication and interpersonal skills.
• Creole/English bilingual abilities strongly preferred.

Responsibilities

• Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management
• Diagnose and treat acute and chronic medical conditions
• Collaborate with multidisciplinary teams to coordinate patient-centered care
• Counsel patients on lifestyle modification, disease prevention, and medication management
• Maintain accurate, timely documentation in the electronic health record (EHR)
• Participate in quality improvement initiatives and team meetings
• Complete additional tasks as directed by supervisor
